Please review all policies below. These are agreed to upon membership sign-up and apply to all members and participants.
Waiver & Physical Accident Release of Liability
I understand that participating and/or competing in amateur Muay Thai involves progressive physical exercise and may involve extreme physical exertion. I waive any possibility of personal injuries which may be attributed to such a program in the future or present, and accept full responsibility for requesting such exercise and competition. I acknowledge that the possibility of certain unusual physical injuries in this martial arts program does exist.
I confirm that I have consulted with my physician and that I am physically fit to participate in martial arts programs. Risks include, but are not limited to: abnormal blood pressure, fainting, disorders of the heart, sprained ligaments, strained muscles, and in rare instances, bone fractures or heart attack. Due to the nature of martial arts programs, I understand that there may be physical contact and I may receive possible punches to the head and upper torso (chest, stomach, ribs, etc.). These punches to the head may lead to injuries such as fractured teeth and/or fractures of the bone structure of the facial area.
I fully understand that kicks and knee strikes are possible to the legs, upper torso, and head area, with possible bruising, cuts, and bone fractures resulting. I also realize that elbow strikes to the head and body are possible and may result in bruising, cuts, and bone fractures.
PHADY'S MUAY THAI INC. shall not be liable for any damages arising from personal injuries or damage which may occur to a member in, on, or about the premises of any schools affiliated with PHADY'S MUAY THAI INC., or any halls, gyms, etc. that may be rented, leased, owned, or borrowed for or by PHADY'S MUAY THAI INC. The member hereby and forever releases PHADY'S MUAY THAI INC., its directors, officers, council, advisors, members, owners, employees, and all other persons participating and associated with PHADY'S MUAY THAI INC., whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the member's use of the said club and its facilities, including but without limitation any claims for personal injuries resulting from the negligence of PHADY'S MUAY THAI INC., its owners, employees, and any people associated with PHADY'S MUAY THAI INC.
I understand that I am required to follow all gym rules at all times. Failure to do so may result in expulsion, membership cancellation, and the inability to participate in any PHADY'S MUAY THAI INC. event. I hereby release and discharge PHADY'S MUAY THAI INC. and all of its affiliates, governing bodies, clubs, coaches, officials, members, agents, officers, and employees from all claims, demands, actions, judgments, and executions which the undersigned's heirs, executors, administrators, or assigns may have or claim to have, for all personal injuries, known or unknown, and injuries to property, real or personal, caused by or arising out of participation in the sport of Amateur Muay Thai.
I, the undersigned, fully understand that this sport activity carries inherent risks. I am fully aware of the nature of these risks, and I voluntarily waive all rights, claims, and causes of action as enumerated herein, and do hereby assume the risk in full. I have read this Release and Waiver, understand all of its terms and conditions, and execute it voluntarily and with full knowledge of its significance.
Gym Hygiene Policy
The health and safety of all members and staff at Phady's Muay Thai Inc. is of the utmost importance. All members are required to adhere to the following hygiene standards as a condition of membership and participation.
Personal Hygiene: All members must arrive to class clean and in fresh training attire. Fingernails and toenails must be trimmed short to prevent injury to training partners. Members with long hair must tie it back securely before training.
Equipment Hygiene: All personal equipment — including gloves, shin guards, hand wraps, and mouthguards — must be cleaned and aired out after every session. Hand wraps must be washed regularly. Members are encouraged to use antibacterial spray on their equipment before and after use. Equipment bags should not be left sealed between sessions.
Gym Matting: Members must not wear street shoes on the training mats. Bare feet or clean training footwear only. Members are responsible for wiping down any shared equipment or surfaces they use during training.
Illness Policy: Members who are feeling unwell, experiencing symptoms of illness, or have a contagious condition (including but not limited to skin infections, ringworm, staph, or respiratory illness) must not attend class until they have fully recovered and, where applicable, received medical clearance.
Failure to comply with the gym's hygiene standards may result in being asked to leave the session and, in repeated cases, suspension or cancellation of membership. Phady's Muay Thai Inc. reserves the right to enforce these standards at its sole discretion.
Cancellation & Refund Policy
Monthly Membership Cancellation: Members who wish to cancel their membership must provide written notice a minimum of thirty (30) days prior to their next billing date. Cancellations requested mid-month will not result in a prorated refund for the remainder of that billing period. The membership will remain active through the end of the current paid month, after which it will not renew. No partial-month refunds will be issued under any circumstances.
Promotional & Discounted Memberships: All promotional and discounted memberships or packages are final sale. Discounts offered through special promotions are provided at a reduced rate, and therefore no refunds will be issued once the purchase has been completed. By accepting a promotional rate, the member agrees to these terms and acknowledges that discounted offers are strictly non-refundable and non-transferable.
General Refund Policy: Phady's Muay Thai Inc. does not offer refunds on membership fees, registration fees, or any purchased gear or apparel. In exceptional circumstances (e.g., a documented medical condition preventing participation), requests for consideration may be submitted in writing to management. All such decisions are made at the sole discretion of Phady's Muay Thai Inc. and are not guaranteed.
Membership Freeze: Members experiencing extenuating circumstances may request a temporary membership freeze. Freeze requests must be submitted in writing and are subject to approval by management. Approved freezes will pause the billing cycle for the agreed-upon period. Freeze requests do not constitute cancellation.
Photo & Video Release
I hereby grant Phady's Muay Thai Inc. permission to use my likeness — and, where applicable, the likeness of my minor child or ward — in photographs, videos, or other digital media ("media") in any and all of its publications, including web-based publications, social media platforms, promotional materials, and marketing campaigns, without payment or other consideration.
This release applies to both adult members and minor participants enrolled in the Tiny Thai's Kids Program. By signing on behalf of a minor, the parent or legal guardian confirms they have the authority to grant this release and do so voluntarily.
I understand and agree that all media captured on the premises of Phady's Muay Thai Inc. or at any affiliated event will become the property of Phady's Muay Thai Inc. and will not be returned. I hereby irrevocably authorize Phady's Muay Thai Inc. to edit, alter, copy, exhibit, publish, or distribute this media for any lawful purpose related to the promotion and operation of the gym.
I waive any right to inspect or approve the finished product in which my likeness or the likeness of my minor child appears. I additionally waive any right to royalties or other compensation arising from or related to the use of such media.
I hereby hold harmless, release, and forever discharge Phady's Muay Thai Inc. from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
Uniform Policy — No Uniform, No Participation
Phady's Muay Thai Inc. enforces a strict uniform policy as a condition of participation. All members — adults and children alike — must arrive to every class wearing the official Phady's Muay Thai gym uniform. This policy is non-negotiable and applies to all training sessions, classes, and gym events.
Required Uniform Items: The official Phady's Muay Thai gym shorts and gym t-shirt or rash guard must be worn at all times during training. Street clothing, non-gym branded shorts, and casual wear are not permitted on the training floor.
Required Equipment: In addition to the gym uniform, all students must have and use their own hand wraps, boxing gloves, shin guards, and mouthguard. These items are mandatory for participation in all classes. Students who arrive without the required equipment will not be permitted to train that session.
Why This Policy Exists: The uniform policy exists to uphold the standards, discipline, and identity of Phady's Muay Thai. It fosters unity among students, demonstrates respect for the art and the gym, and ensures a safe and professional training environment for everyone on the mats.
New Members: Uniform items and required equipment are available for purchase through the gym. New students are encouraged to contact us prior to their first class to arrange their uniform so they are ready to train from day one.
Failure to comply with this policy will result in the student being asked to sit out that session. Repeated non-compliance may result in suspension or cancellation of membership at the sole discretion of Phady's Muay Thai Inc. management.
